PS2 Slim - Scratching the disc! Need Help!

I got this prob lately. It scratches sort of in the same area "ring shape". Thank god it doesnt happen 2 all the disc but most already kena. I always ensure I put in the disc properly before running it but still it happens.

Mine is the slim type n its standing up. Wats the problem? Laser arm? Disc holder? Any advice is appreciated.
